A mother who died during a horrific crash on a NSW motorway with her two children might have made a deadly error.
A relative has spoken out, saying Katrina Prahastono, 34, was chatting to a good friend on speakerphone and had missed a turn-off just before her SUV collided with a cement truck on the Hume Motorway at Menangle Park final week.
